About the Report Formats
- Each reporting form is available in four file formats:

- PDF documents can be viewed and printed using the free Adobe Acrobat Reader.
- Although you can print blank forms using Adobe Acrobat (PDF), this format does not allow you to enter patient information electronically. The patient information must be entered manually on the printed blank forms. The physician must sign in the appropriate item box.
- WordPerfect and Microsoft Word can be used to electronically enter the patient information. To electronically enter the patient information, set mode to ‘overtype'. Type in patient information in item boxes. For the item boxes with choices offered, use the mouse to click on the line preceding each item you want to select for this patient/abortion; then place an "x" on that line. Print the completed form and get physician's signature in the appropriate item box.

DO NOT USE A WORD PROCESSOR FORM IF IT PRINTS INCORRECTLY.
Instead, print the form using Adobe Acrobat. Alternately, you can call the Vital Records and Health Data Development Section at (517) 335-9826 and request an order for blank forms.
Downloading
WordPerfect and Microsoft Word documents are stored in a self-extracting zip file. Here are the simple instructions for downloading the zip files:
- Click on the link that refers to the zip file.
- If the browser asks you what you want to do with the file (the Netscape browser usually will), select Save File.
- Specify your Desktop or a designated folder (directory) as the destination.
- After the download, find the zip file on the Desktop or designated directory.
- Click on the file to run it (to make it unzip itself).
- The default destination directory for the content files is probably set to C:windowsdesktop (the desktop in Windows95). Accept that or change it to the desired destination folder.
- The content files should be found on the Desktop or in the folder, ready to use.
