PDG Home Visiting Transitions Mini QI Learning Collaborative Results & Resources
During 2022-23 through the Michigan Department of Education’s Preschool Development Grant three local home visiting teams came together to test strategies for supporting smooth transitions for families between home visiting programs and early childhood settings. The Home Visiting Transitions Mini QI Learning Collaborative work of these three teams provided immense learning around what transitions look like at the local level, direct feedback from parents on their experiences, and the supports and strategies tested and found beneficial to staff and families. Thank you to Healthy Families District Health Department (DHD) #10, Oakland Local Leadership Group (LLG), and the Ottawa Maternal Infant Health Program (MIHP) & Parents As Teachers (PAT) teams!
