<strong">Payable Benefit - Dependent Life</strong">

Employees have the option of enrolling their legal spouse, and/or children in dependent life insurance coverage through Minnesota Life via payroll deductions. Employees may enroll through the MI HR Service Center* upon hire, within 31 days of a life event, or during the annual open enrollment period. The following are the options available to you for dependent life insurance coverage:

Option 1 - $1,500 for spouse & $1,000 for dependents
Option 2 - $5,000 for spouse & $2,500 for dependents
Option 3 - $10,000 for spouse & $5,000 for dependents
Option 4 - $25,000 for spouse & $10,000 for dependents
Option 5 - $10,000 for dependents only

New for the 2016-2017 Plan Year:

Option 6 - $50,000 for spouse & $15,000 for dependents
Option 7 - $15,000 for dependents only

Note: MSPTA (T01) represented employees are ineligible for Dependent Life Insurance Options 6 and 7.

Dependent Life Insurance is an optional plan and is paid by the employee via payroll deductions. Visit the Insurance Rates page to see what each option will cost per pay period.

Employees will need to contact the MI HR Service Center* to file a claim if the dependent was enrolled in dependent life insurance. Employees have the option of enrolling their legal spouse, and/or children in dependent life insurance coverage, with the employee automatically designated as the beneficiary.

Eligibility for Dependent Life Insurance

Dependent and spouse eligibility is based on the following:

Your legal spouse Yes

Your children by birth, legal adoption, or legal guardianship who are:

  • Between the ages of 14 days and 23 years
  • Unmarried
  • Dependent on you for at least 50% of their support

Yes

Any other unmarried child between the ages of 14 days and 23 who:

  • Lives with you
  • Is dependent on you for at least 50% of their support

Yes

A spouse who:

  • Is eligible for insurance under the policy as an employee or retiree
  • Is your ex-spouse

No

A child who:

  • Is less than 14 days old
  • Has attained the limiting age of 23
  • Is in the armed forces of any country (except for active duty of two weeks duration or less)
  • Is enrolled in State group life insurance under the policy as an employee, retiree, or a dependent.
  • Has been adopted by another person
  • Is married

No

If you are unsure if your dependent can be covered, further information on dependent eligibility can be found in the Minnesota Group Life Insurance Booklet.

Your child can only be insured as a dependent of one parent if both you and your spouse are insured under the policy as State of Michigan employees. In the event you cannot agree which parent will cover the child(ren), the parent whose birthday falls closest to the first of the calendar year will cover the dependent child(ren).
