Use EyeMed's Provider Locator Tool to find In-Network Providers in your area. When using the tool, leave the default network set to "Access." This is the title of the State Vision Plan network, so changing the network setting to anything but "Access" may result in an incorrect list of providers.
If you are a seasonal, part-time, permanent intermittent or job sharing employee, or if you are a State employee married to another State employee/retiree, see the Employee Eligibility page. There is a distinction between benefits payable to EyeMed's participating providers and non-participating providers. Participating providers will file your claim for you and, aside from any non-covered options you order, they will accept EyeMed's payment as payment-in-full. However, if you use the services of a non-participating provider, you must pay the provider yourself and file your own claim for a partial reimbursement in accordance with the plan design for this benefit, which is outlined in the summary and guides above. *Except for those employees in certain bargaining units who are hired on or after 1/1/00 who have a regular work schedule of 40 hours or less per bi-weekly pay period (not permanent-intermittent positions) or Deferred Retirement Option Plan (DROP) employees.
$1,000 Lasik reimbursement is extended from employee only coverage to also include coverage for a spouse of an active employee, effective 1/1/2023*. This is a lifetime maximum benefit and applies only to employees and their spouses; dependents who are not benefit-eligible state employees are not covered.
To request reimbursement for Lasik procedures performed, eligible individuals* should complete the Out-of-of Network Claim Form, accessible via the prior link, or by logging in to the EyeMed State Vision Plan Member Portal. Select "Claims", then the "Out-of-Network Claims" tab, and complete the Out-of-Network Claim Form for reimbursement. Eligible individuals will also need to include an itemized receipt for Lasik services with the claims form. If you are an eligible state employee enrolled in the State Vision Plan as another state employee's dependent, call 1-833-279-4355 to ensure enrollment prior to submitting the Out-of-Network Claim Form.
*Employees enrolled in the State Vision Plan are eligible as of 10/4/2020 to seek reimbursement, MSPTA-represented (T01) employees enrolled in the State Vision Plan are eligible to seek reimbursement for dates of service on or after 1/1/2023. Spouses covered under an employee's State Vision Plan are eligible to seek reimbursements for dates of service on or after 1/1/2023.
