Long Term Disability (LTD)
The benefits described in the Long Term Disability Plan Summary are provided under the State of Michigan's self-funded Long Term Disability Plan.

Short Term Disability (STD)
(Michigan State Police Employees Only)
Reliance Short Term Disability
This Short Term Disability (STD) plan is available only to Michigan State Police employees who are under the age of 60 at the date of application. Reliance STD allows you to receive a weekly income benefit if you are disabled due to sickness or injury and become disabled while insured by the policy, unless it is due to a pre-existing condition. You can enroll within 31 days of your hire date by contacting the MI HR Service Center at 877-766-6447, 8:00a.m. - 5:00p.m., Monday through Friday. You may also enroll during the annual Benefits Open Enrollment period through HR Self-Service.
Your benefit choice will continue if no changes are made. An employee wishing to cancel their insurance coverage must complete and submit the Reliance - Cancellation form, submit a signed & dated letter stating that you wish to cancel the coverage, or submit an email from your State of Michigan email address stating that you wish to cancel the coverage.
