Pet Insurance from MetLife provides coverage to employees for their beloved pets. Raising a pet can be a joyous experience but providing care for them can quickly become expensive. Pet Insurance can help by reimbursing you for covered veterinary expenses.

Plan Features

Pet Insurance provides reimbursement for vet visits due to accidents and illnesses. Coverage is available for cats, dogs, birds, and exotic animals1. View the Covered Species List to see pets eligible to be covered under this benefit. This benefit allows flexibility to customize a plan with the following coverage levels:

There are plan additions like optional wellness available5, which provides reimbursement for preventive care. It covers services or procedures for the purpose of prevention of injury or illness or for the promotion of general health. In other words, it can cover things like vaccinations, teeth cleaning, wellness exams, heartworm tests, blood panels, and more.

Employees with more than one pet have the option to make their plan a family plan6. Family plans will cover up to three pets on the same policy using the same selected coverage levels and plan limits.

Rates & Enrollment

Enrollment in the plan is available year-round, first starting with the Benefits for Life Open Enrollment in March of 2024. Additional plan information and the enrollment link can be found by visiting benefitsforlife.org, locate the section on the right side of the page "Benefits for Life (BFL) State of Michigan Voluntary Benefit Program for Active Employees," and then select the drop down "Pet Insurance - MetLife".

Every pet and pet parent is unique, so each pet’s premium will be unique. Rates are based on several factors including species, breed, age, zip code, riders selected by you, coverage amounts elected, and available discounts. Premiums can be paid by payroll deduction, direct bill, or credit card. Your online quote for Pet Insurance will reflect a monthly premium. Please note, if you choose payroll deduction as your preferred payment method, deductions will be withheld biweekly from your State of Michigan pay check. The biweekly premium is calculated by multiplying the monthly amount by 12 months, then dividing that by 26 pay periods.

File a Claim

Simply take your pet to any licensed veterinarian and pay at the time of service. Within 90 days from the date of service, submit your claim to MetLife via mobile app, online portal, email, fax, or mail. Your claims will be reimbursed by check or direct deposit if the claim expense is covered under the policy and according to the plan limits and coverage you elected.

 

1Exotic pet reimbursement options include: 50%, 70%, 80% and 90%. For exotic pets, deductible options are $0-$2,500. The maximum annual limit offered for exotic pet coverage is $10,000. Unlimited benefit option is not available for exotics. Optional Preventive Care coverage is not available for exotic pets.

2 Reimbursement options include: 50%, 70%, 80%, 90% and 100%. 100% reimbursement is limited to no Wellness and an annual maximum of no more than $5,000. Exotic pet reimbursement options include: 50%, 70%, 80% and 90%.

3Deductible options range include: $0 -$750 in $50 increments and $1,000, $1,250, $1,500, $2,000 and $2,500. For exotic pets, deductible options are $0-$2,500.

4Annual limit options range from $1,000 -$25,000 in $1,000 increments. There is also a $500 annual limit option for MetGen policies. Unlimited benefit option is not available for exotics. The maximum annual limit offered for exotic pet coverage is $10,000. Unlimited benefit option cannot be selected online, employees must call to elect that option.

5Optional Preventive Care coverage is included in the annual limit. Not available for exotic pets.

6MetGen family plan policies are limited to dogs under the age of 13 and cats under the age of 15. Multi-policy discount is not available with Family Plans. Exotics will need to be on separate policies.

7When using multiple discounts, discounts cannot exceed 30%. Each discount may not be available in all states. Please contact MetLife Pet for further details. Multi-policy discount is not available with Family Plans.
