WHEREAS, case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet health needs through communication and sharing available resources to promote individual independence, safety, quality of care, and cost-effective outcomes; and,
WHEREAS, when an individual reaches the optimum level of wellness and functional capability, everyone benefits, including the individuals being served, their support systems, the health care delivery systems, and the various reimbursement sources; and,
WHEREAS, case management evolved as a practice in the early 1900s; the current evolution of formalized case management began with Medicaid and Medicare in the early 1970s; and,
WHEREAS, today, case management is a multi-disciplinary profession and an essential component of health care; case management has proven its worth by improving rehabilitation, quality of life, client satisfaction, and compliance with a person-centered approach while promoting self-determination and reducing healthcare costs;
NOW, THEREFORE, I, Gretchen Whitmer, governor of Michigan, do hereby proclaim October 8-14, 2023, as Case Management Week in Michigan.
