Step I: Fingerprint Scan
Former law enforcement officers applying for initial LEOSA certification must complete a fingerprint-based criminal history check. Fingerprints must be submitted electronically through Live Scan (print cards are not accepted).
Fingerprinting is available through:
- Local sheriff’s offices
- Local law enforcement agencies that offer applicant fingerprinting
- Approved private Live Scan vendors (available above)
- Michigan State Police does not provide fingerprinting services
Before your appointment:
- Confirm the location provides applicant fingerprinting
- Use Reason Code: RLE (not CCW)

Required form:
Complete the RI-030 Live Scan Fingerprint Request Form before your appointment. To obtain the form, contact the Commission at 517-636-7868.
After fingerprinting:
Include the original, completed fingerprint request form with your LEOSA application packet.
Step II: Complete the Application
A complete application consists of:
- Completed and signed application
- Former employer letter on official agency letterhead and signed by an authorized agency representative. The letter must follow the MCOLES Former Employer Letter template. Letters that do not follow the template will not be accepted.
- Passport-quality photo taken within the last six months, with a clear, front-facing view, white background, and no hat or dark glasses. A new photo is required every five years for renewals.
- Copy of original proof of fingerprinting
- Copy of a valid driver’s license or state-issued ID
Step III: Application Review and Certification
The Commission will review the application and confirm eligibility requirements are met.
Once all materials are received:
- Training/test results from the regional Training/Test Center will be submitted directly to the Commission.
- Applicants will be notified by email upon approval.
- First-time applicants will receive a LEOSA certificate/identification card valid for one year.
- After the initial one-year period, applicants wishing to continue their certification should follow the renewal steps.
