Midwestern Dental End of Operations

Midwestern Dental has informed the state that they are permanently ceasing operations and closing all locations due to the economic challenges and uncertainties associated with COVID-19. As a result, Midwestern Dental will no longer offer dental insurance benefits to State of Michigan employees or their dependents after May 29, 2020.

This was unexpected, and we understand the frustration and difficulties it may cause. To ensure continuity of coverage, employees enrolled in Midwestern Dental have been automatically enrolled in the State Dental Plan, through Delta Dental, effective May 30, 2020. If employees wish to continue coverage in the State Dental Plan, no further action is required. To enroll in the State Preventive Dental Plan or waive coverage, employees will have until June 30, 2020 to contact the MI HR Service Center to request the change. Failure to contact the MI HR Service Center (877-766-6447, Monday - Friday, 8:00 a.m. - 5:00 p.m.) by this deadline will result in continued coverage in the State Dental Plan until the next opportunity to make a change during the upcoming open enrollment period or another qualifying life event occurs.

All Midwestern Dental billing related matters should be directed to their Central Billing Office at 732-750-5229 or 732-750-5228. Employees needing to obtain copies of dental records or treatment plans from Midwestern Dental must contact their former Midwestern Dental office of record and follow the phone prompts to request a copy.
