If you add new dependents to your insurance coverage during BOE, the MI HR Service Center1 must receive your emailed, mailed, or faxed proof of eligibility documentationby November 13, 2023 for the enrollment to be valid. Click the links below to view a listing of valid documents and requirements.
State Police Employees Only: The MI HR Service Center must receive your mailed or faxed Evidence of Insurability form and Life Insurance Enrollment form no later than November 13, 2023.
