Midwestern Dental permanently ceased operations and closed all locations. As a result, Midwestern Dental no longer offers dental insurance benefits to State of Michigan employees or their dependents as of May 29, 2020.
Employees previously enrolled in Midwestern Dental were automatically enrolled in the State Dental Plan, through Delta Dental, effective May 30, 2020. Employees who wished to enroll in the State Preventive Dental Plan or waive coverage were required to contact the MI HR Service Center to request the change by June 30, 2020.
Dental coverage may be selected or changed during the annual Benefits Open Enrollment (BOE) period each fall or within 31 days of a qualifying life event (QLE) by contacting the MI HR Service Center* at 877-766-6447, Monday through Friday, 8:00 a.m. – 5:00 p.m.
*Auditor General and Judicial employees should submit all supporting documentation to their HR office instead of the MI HR Service Center.
