Utilization Review Orders

This page contains official utilization review (UR) orders issued by the Michigan Department of Insurance and Financial Services (DIFS) under Section 3157a of the Insurance Code of 1956 (Code), 1956 PA 218, MCL 500.3157a.

Utilization review orders are formal determinations related to disputes over medical treatment and services provided under Michigan’s no-fault auto insurance law.

Each UR order was based solely on the documentation submitted by the parties for review and applies only to the treatment and date(s) of service specifically addressed therein. Previously adjudicated orders may not be relied upon by any entity to determine an injured persons’ eligibility for treatment or as a basis for action regarding treatment or dates of service not expressly addressed in that order.

Each UR order is provided as a PDF document and includes information about the:

  • Health care provider
  • Auto insurer or Michigan Catastrophic Claims Association (MCCA)
  • Medical service or treatment in question
  • Independent review organization’s findings, if applicable
  • Amount payable for the date of service, If applicable
