  • Custody paperwork – if applicable
  • Current IEP/504 – if applicable
  • Insurance card (front and back)
  • Legal follow up – if applicable
  • Hospitalization discharge paperwork – if applicable
  • Parent ID
  • Shot records or shot waiver
  • Youth’s social security card
  • Youth state ID

If you do not have electronic copies available copies will need to be emailed to DMVA-Apply-MYCA@michigan.gov as a PDF attachment. No other formats will be accepted.
