STC Form 2164 (L-4023) and Form 2795 (L-4046) for each county for 2007 and prior can be provided by Freedom of Information Act (FOIA) request. Requests should include the county(s) and year(s) you are requesting and must include a name, address, and contact information (phone number or email address).
