  • Demographic changes can be entered for all agencies regardless of the license status.

  • Documentation (legal proof of the name change/DBA) is required for an agency name change or addition of a DBA.

  • An agency name change and addition of a DBA are not effective until approved by DIFS.

  • Business addresses cannot be post office box addresses.
