Duplicate Certificate of Authority
To request a duplicate Third Party Administrator (TPA) certificate of authority, submit an email request to DIFS-LicensingORE@michigan.gov using TPA Duplicate Certificate of Authority as the subject line. In the body of the email, provide the exact name of the TPA, the TPA’s 7-digit Michigan System ID number if known, and the TPA’s mailing address for verification. The certificate of authority will be mailed to the mailing address. Please allow up to three weeks for receipt. Note that only active certificate holders may receive a duplicate TPA certificate of authority. A TPA certificate of authority cannot be sent via fax or as an email attachment.
No payment is required for a duplicate certificate of authority.
If you do NOT need a duplicate TPA certificate of authority and only need to check the status of a TPA, use the DIFS Locator. The certification status of a TPA can be viewed and printed via the Locator: DIFS Locators . Information on the Locator is updated in real-time and can be used as proof of certification.
