Exhibits described in and submitted with your application are tools you can use to prove your financial, managerial, and technical ability to provide local exchange service to customers in Michigan. Exhibits may also be used to show that licensing your company to provide local exchange service is not contrary to the public interest. The exhibits accompany the expert witness testimony, and are explained in that direct testimony. Expert testimony is a required component of the complete application.
Exhibits May Include Some or All of the Following
Required:
- Financial Information that Directly Relates to the Applicant (e.g., Annual Reports, Balance Sheets, Form 10-K, Form 10-Q, Income Statements, and/or other.)
- Biographies of Company Officers Emphasizing Telecommunications Experience
- Illustrative Tariffs or adoption of the Commission’s generic tariff by reference language
- Maps and Legal Descriptions of Exchanges, or Maps and Legal Description Mirroring Language
- Certificate of Incorporation or Certificate of Authority to Transact Business in Michigan if incorporated out of state
