How do I determine my Annual Financial Statement (AFS) reporting period?

The reporting period is determined by the date the licensee was issued its first medical license. The AFS report must cover all medical licenses held by the licensee at any time during the reporting period. Licensees will receive notification by email approximately 6 months prior to the due date of their AFS report, reminding them of their AFS requirement. The notice will include the licensee’s assigned reporting period for that fiscal year. More information regarding reporting periods can be found in the AFS advisory bulletin for the fiscal year.
