How should I submit my Annual Financial Statement (AFS) report?

The medical AFS report must be uploaded to the licensee's medical prequalified ERG record in Accela Citizens Access Portal (ACA) or sent via email to CRA-AFS@michigan.gov.

  • If uploading to the licensee's ERG record, use the "AFS Documents" document category.
  • If sending via email, include the licensee's name, and ERG # in the subject line.
