The medical AFS report must be uploaded to the licensee's medical prequalified ERG record in Accela Citizens Access Portal (ACA) or sent via email to CRA-AFS@michigan.gov.
- If uploading to the licensee's ERG record, use the "AFS Documents" document category.
- If sending via email, include the licensee's name, and ERG # in the subject line.
