How do I reapply if my paper application is denied?
A new application and all supporting documents are required with all reapplications.
It is important that the patient read and follow the instructions in the denial letter.
The patient cannot just submit the missing or corrected information. Please refer to the bottom of the denial letter as to whether a new $40 application fee is due or remains on file.
The patient’s payment remains on file for six (6) months from the date the payment was originally received.
Patients/caregivers should date the new application on the date they physically sign it – do not backdate or use the same date that was on the denied application.
