I have an active patient registry identification card and I recently changed my name or moved. How do I update my name or address on my registry card? How much is the fee?

Prior to your renewal period (more than 90 days before your card is due to expire):
You must submit the Change Name or Address Amendment online or via the paper form by mail.

To change your name or address online, you must be a patient, visit our website at www.michigan.gov/mmp for instruction and the online portal.

If your Amendment is approved we will re-print all your active registry cards.
