Paper Applications, Forms and Resources

    • Application Instruction Booklet

      The Cannabis Regulatory Agency (Agency) is utilizing a two-step application process for state operating licenses in Michigan. The two-step process consists of prequalification and facility licensing.  Please consult the Application Instruction Booklet to ensure that the application has been completed and provided to the Agency in the manner required.   Failure to do so may delay the processing of the application.

       

    • Applicant Entity Prequalification Application (AEPA)

      Entities wishing to apply for a state operating license must first complete this prequalification application and provide required supporting documentation.  A non-refundable application fee of $3,000.00 must be paid at the time of filing of the prequalification application.

       

    • Supplemental Entity Prequalification Application (SEPA)

      Entities that have an ownership interest in an applicant entity must complete this prequalification application and provide required supporting documentation. 

       

    • Sole Proprietor Prequalification Application (SPPA)

      An individual wishing to apply for a state operating license must first complete this prequalification application and provide required supporting documentation.  A non-refundable application fee of $3,000.00 must be paid at the time of filing of the prequalification application.

       

    • Supplemental Individual Prequalification Application (SIPA)

      Individuals that have an ownership interest in an applicant entity must complete this prequalification application and provide required supporting documentation. 

       

    • Facility License Application (FLA)

      Applicant entities and sole proprietors that have identified a location from which to operate a proposed marijuana facility must file a facility license application and provide required supporting documentation.  A prequalification application must first have been submitted.  A facility license application must be filed for each location and license type for which the applicant wishes to apply.
