Before submitting your application, please have your supporting documentation ready for at least one of the following program criteria:
 

Residency in a Disproportionately Impacted Community

When applying for the residency fee reduction, we may accept many different documents.  To be eligible for the residency fee reduction, the applicant must have lived in at least one of the disproportionately impacted communities identified by this program for at least 5 cumulative years.   For a list of documents we may receive and more information, please see the instructions linked below. Marijuana-Related Conviction

When applying for a marijuana-related conviction fee reduction, an applicant may submit documents such as the Judgment of Sentence.  Documents should include the applicant’s name, date of birth, the name of the conviction, and the date of the conviction.  If the conviction has been expunged, an Order on Application to Set Aside Conviction may be accepted.   For more information, please see the instructions linked below.

Registered Primary Caregiver for at least 2 years of the last 5 years

When applying for the caregiver fee reduction, the applicant will complete and notarize the form linked below.  In submitting the form, the applicant will include a copy of their driver’s license or State ID card.
