Application Package

The Michigan Youth Challenge Program application has several parts. All components are necessary to make a proper determination and to admit candidates to the program.

There are two ways to apply:

1. Complete the basic Application and required disclosures via your web browser (please use a secure internet connection and not public WiFi to protect your privacy). Next, download the forms that require printing and ink signatures (listed below). Once the forms below are complete, please scan and save as a PDF file and send them as an attachment via email to DMVA-APPLY-MYCA@michigan.gov with your applicant's name in the subject line.

• Youth Questionnaire and Applicant contract - distributed and collected at Discover MYCA Information Session
• Copy of current, unofficial, academic transcripts

2. Download the application package (PDF Fillable forms), complete all pages, save as a PDF file, and send them as an attachment via email to: DMVA-Apply-MYCA@michigan.gov with your applicant's name in the subject line.

Once the application is received, you will receive a checklist (via email) outlining any additional supporting documentation that will be required to make the selection determination. As documents are received and reviewed by MYCA staff, your checklist will be updated and resent to you to keep you aware of your current progress in the process.
