Current Disciplinary Action Reports for the Health Professions Division

To comply with this requirement, the Health Professions Division in the Bureau of Health Care Services frequently publishes a Disciplinary Action Report (see below). The Disciplinary Action Report (DAR) lists the disciplinary actions taken against health professionals who are licensed and regulated by the various health boards within the Health Professions Division. The report also includes updated information regarding licensees who have appealed the Board's action to a higher court. The report includes the names of the health professional, their professional license number, the type of disciplinary action taken, the effective date of the action and the general nature of the complaint.

You may also fax your request to 517-241-1212. Note: Please submit your request to us in only one format.
