The following template is provided by Michigan Public Service Commission (MPSC) Staff for disclosure of supplier specific and regional fuel sources used to generate electricity. This standardized template has been prepared as directed by the Commission in its orders in Case No. U-12487, dated June 5, 2001 and December 20, 2001, concerning customer disclosure standards as required under Section 10r(1) of Act 141 PA 2000.
