Please complete the survey by 6 p.m. Friday, March 1. Your responses are confidential and will be used for planning and statistical purposes. The record kept of your survey responses does not contain any identifying information about you unless you provide answers to questions that specifically ask for this information. However, it is very helpful for us to make projections of future customer needs when you provide demographic information at the end of the survey.
