Prepaid Funeral and Cemetery Contract Provider registrations come due for renewal every three years. A renewal notice will be mailed to the last address on record approximately 60 days prior to the expiration date.

A registrant may reinstate the registration within 60 days of its expiration by paying a $120.00 fee.
