The MLCC forms are presented in Word (.doc), Excel (.xls) or Adobe (.pdf) format. They are available for viewing with either the Microsoft or Adobe Acrobat Readers. If you have not downloaded files in Acrobat, Word, or Excel before you must first download one of the free readers available from Adobe or Microsoft. NOTE! Adobe forms cannot typically be saved, you must fill out the form then PRINT IT, SIGN IT (where applicable) and FAX IT to the appropriate division if FAX submission is permitted. If attachments are included it is best to US mail all items together as one packet. Download viewer from Adobe.
