Avoid Issues with Proof of Age
Not providing the correct proof of age for you, your spouse, or your dependent could cause delays in processing your retirement application or insurance coverage.
Common issues we see related to providing proof of age include:
- Document is unreadable.
- Document is not the correct (acceptable) type.
Tips to avoid these issues:
- Government-issued birth certificate, or valid passport, or valid driver's license are preferred.
- Make a copy. If it copies well, it should also fax well. If copy quality is poor, it will not fax well. Do not send originals; they will not be returned.
- Provide proof of age for anyone listed as a beneficiary or dependent on your insurance.
