BILLING & PAYMENT:

Invoices issued after delivery of shipments.
Check payments mail to: Michigan State Industries, 206 E. Michigan Ave, Lansing, MI 48933.
MDOC staff use SIGMA or IET payment process form
State Agencies use SIGMA or SOM IET payment process form

INVOICES:

Invoices sent via e-mail from AUTO-FORMS@MICHIGAN.GOV
Please include current email address on orders to receive prompt receipt of invoice.

IMPORTANT DELIVERY INFORMATION!!

SHIPPING DISCREPANCIES INCLUDING "OVERAGES", "SHORTAGES" or "WRONG PRODUCT RECEIVED"
MUST BE REPORTED VIA EMAIL TO MSICUSTOMERSERVICE@MICHIGAN.GOV WITHIN 2 WEEKS OF RECEIPT.
FREIGHT SHIPPING PROGRAM includes FREE DELIVERY using the following criteria:

Delivery order is over $100.00. NOTE: Orders under $100 will be charged a $10.00 freight fee.

Delivery is within the State of Michigan.

Delivery is via MDOC freight trucking.

Delivery is drop shipped to a loading dock or secured area 50 feet from the delivery entrance of the customer's "ship to" address.

All items are in one delivered trip.

Orders shipped Out-Of-State will be assessed shipping charges via a common carrier.

Delivery does not include removal of packing material.
