Request DD-214 Separation and Discharge Documents

To ensure timely processing of your request, please carefully review and follow the instructions below. Incomplete or incorrect submissions may result in delays or denial.
Required Identification
All online requests must be accompanied with a valid form of photo identification for the veteran submitting the request. Acceptable forms of ID include:
- State-issued Driver's License
- State-issued Identification Card
- Employment ID with Photo
- Federal or State Government-issued ID
- U.S. Passport
- Military or Veteran ID with Photo
Third-Party Requests
If you are not the veteran, please include the appropriate documentation:
For Living Veterans (unable to sign):
Submit a valid Power of Attorney (POA) or VA Form 21-22. Be sure to include:
- The page identifying the authorized individual
- All signature pages of the POA
For Deceased Veterans:
Submit one of the following:
- A copy of the veteran's death certificate that clearly identifies the requester
- A published obituary that clearly identifies the requester
Note: Funeral homes are not authorized to submit requests on behalf of the family unless they are specifically listed on the death certificate.
Failure to include the appropriate supporting documentation may result in processing delays or denial of your request.
Processing Time & Notification
- Please allow up to three business days for processing
- If your document(s) is being sent by ground mail, please allow up to ten business days for delivery
- If the requested record is not on file with the State of Michigan, you may be referred to the appropriate state or the National Personnel Records Center and may need to complete a Standard Form 180 (SF-180)
- Veterans with a Premium eBenefits account can also access their DD214 through the eBenefits portal: eBenefits Access Your Records
