Camp PROGRAM application instructions

Dear Camp PROGRAM Applicant:

This informational packet will provide the following:

  • Camp PROGRAM license application procedure
  • Application Fee procedure
  • Livescan Fingerprinting requirements for certain individuals.
  • Assignment of a Camp Licensing Consultant.
  • Policy and Procedure requirements.
  • Staff and volunteer Criminal History Check No Fingerprint Instructions.
  • Onsite Review if necessary.

Camp PROGRAM and SITE applications are licensed separately. If you as a PROGRAM license rent a Site, you are responsible for making sure the SITE is licensed. Camp administrative rules Part 1, General Provisions, applies to the PROGRAM license. Licensed PROGRAM’s are either a Children’s Residential, Adult Residential, or a Children’s day classification.

PROGRAM Application:

Please review, complete, and submit the PROGRAM application. The PROGRAM application must be submitted each year by May 1st for timely license issuance. The application is required to schedule an annual inspection, maintain accurate licensing data, and to ensure ongoing compliance.

PROGRAM Application Fee:

An application fee must be paid for a new license.

Criminal History Check (CHC) Livescan Fingerprint:
Person’s applying, Licensee Designee’s, Chief Administrators, and Program Directors are required to complete Livescan fingerprint CHC. Instructions to comply with this requirement can be found at, locate the Resources section, and select Criminal History Check Livescan Fingerprint Instructions. If you are an administrator for both a Program and Site license, please indicate both license numbers on your clearance forms.

Camp consultant assignment:
A Camp Licensing Consultant will be assigned to your application once the licensing unit has determined the application is complete, the fee is paid, and CHC Livescan Fingerprinting of required individuals is done. The Camp Licensing Consultant will then ensure compliance with the following:

  1. Public Act No. 116 of the Public Acts of 1973, as amended and Licensing Rules for Children’s and Adult Foster Care Camps.These documents can be found at in the Rules & Statutes section.

  2. Camp policies and procedures as listed at in the Resources section titled Sample Policies, Procedures and Forms.

  3. Compliance verification of staff and volunteer consistent with CHC requirements located at in the Resources section titled Criminal History Check No Fingerprint Instructions.

  4. Once review of items 1,2, and 3 are complete an onsite inspection may be scheduled.
