Abuse and Neglect History Check

Follow the steps below to request an Abuse and Neglect History Check from Alabama:

The Child Care Provider Must:

1. Click on the link to the (AL CAN) Child Abuse & Neglect form.
2. Print the form.
3. Click on the link for Alabama’s specific instructions on how to complete and mail the form (AL CAN) Instructions. Alabama will reject forms that are not completed per their linked instructions.
4. Requesting Person or Agency/Organization: This section MUST be completed by the employer and contain the facility/employer's contact information. *Please note that both the employer signature and a witness signature are required here.
5. The employer should enter the applicant's name and identifying information. Please be sure to complete this section thoroughly.
6. The applicant will need to sign and date the "To Be Completed by Person Being Cleared" Section - and a witness signature will also be required here. *Note that the witness signature in this section cannot match the applicant signature or the employer's signature above.
7. Employer should mail the completed form (must be the original copy) to:

Criminal History Check

Alabama requires the submission of fingerprints to perform a Criminal History Check and charges a $25 fee. Complete the request form and send it with your fingerprints and payment to Alabama by mail.

Follow the steps below to request a Criminal History Check from Alabama:

1. Click on the link to the (AL CH) Criminal History form (Instructions are included and must be read and followed).
2. AFFIDAVIT FOR RELEASE INFORMATION: MiLEAP-CCBC Unit, PO Box 30315, Lansing, MI 48909-7815

Alabama should send the results to the CCBC Unit by mail in about 4-5 weeks. However, if the applicant or provider receives the results of the Criminal History Check, they must send the results to the CCBC Unit by either:

NOTE: The Alabama Law Enforcement Agency APPLICATION TO CHALLEGE Alabama Criminal History Record Information should only be completed if the applicant wishes to challenge the results of their Criminal History Check.

Abuse and Neglect History Check

1. Click on the Adam Walsh Request form.
2. Complete the form and add the following information:

3. Print and sign the form to pen and paper and date it.
4. Make a copy of your photo ID and submit with the request form.

Criminal History Check                                                                                  

Alaska performs name-based Criminal History Checks for childcare purposes and charges a $20 fee.

Follow the steps below to request a Criminal History Check from Alaska:

1. Click on the following link to the (AK CH) Criminal History form
2. Complete the form: Select #2 for "Type of Information requested"
3. Mailing address to send report:

4. Print the request form, sign and date the form at the bottom of page 1

Alaska should send the results directly to the CCBC Unit within 2-3 weeks. However, if the applicant receives the results, they must forward the results to the CCBC Unit by either:

Abuse and Neglect History Check

Follow the steps below to request an Abuse and Neglect History Check from American Samoa:

1. Write a letter to the American Samoa for a Child Abuse and Neglect Registry Check.
2. In your letter include: your complete address, full name, alias names, full date of birth and full social security number.

American Samoa sends the results within several weeks.

When/If the applicant or provider receives the results of the Child Abuse and Neglect History Check, they must send the results to the CCBC Unit by either:

Criminal History Check

The CCBC Unit Analyst will complete the following:

1. Write a letter to the American Samoa requesting a Criminal History Check.
2. Include the CCBC email address to return the results.
3. Mail the letter to: Commission of Police Department of Public Safety, Attn: Le’I Sonny Thompson, Commissioner of Police, P.O. Box 53, Apia, AS 00917.

American Samoa sends the results to the CCBC Unit by email within several weeks.

Follow the steps below to request an Abuse and Neglect History Check from Arizona:

1. Click on the (AZ CAN) Child Abuse & Neglect and locate form 1083a.
2. Complete all sections of form 1083a (leave DCS section blank).

Criminal History Check

The CCBC does not require a separate Criminal History Check from Arizona due to this state’s participation in the National Fingerprint File (NFF). No further action is needed.

1. Obtain the Child Abuse and Neglect History Check for this state as described below, AND
2. Obtain the Criminal History Check for this state as described below.

Currently, there is a $11 fee for processing the AR Child Abuse and Neglect Registry Check and a $25 fee for the processing of the AR Criminal History Check for a total of $36.

Abuse and Neglect History Check

Follow the steps below to request an Abuse and Neglect History Check from Arkansas:

There are three stages involved with receiving results for an Arkansas (AR) Child Maltreatment Central Registry Background Check:

Stage 1: Arkansas (AR) Child Maltreatment Central Registry Form Generator Process

Stage 2: Arkansas (AR) Child Maltreatment Central Registry Form Upload Process

1. Open the form in your email and save it to a place on your computer where you can easily locate it.
2. Once you have your form saved, access the Central Registry Form Uploader to send your file to the Central Registry Team using the links below:
   • You can upload a single request here.
   • You can upload multiple requests (up to 10 requests) using our multi-submission here.
3. When you open the submission link, select the “You are an out of state individual, provider, school, or state agency” bubble and complete the remaining fields below. Please enter in the following information under the RESULTS SHOULD BE RELEASED TO section:
   • Company/Requestor: State of Michigan-MiLEAP-CCBC
   • Contact Name: CCBC Unit
   • Contact Email Address: MiLEAP-CCBC@michigan.gov
4. The APPLICANT INFORMATION section should be completed thoroughly and accurately before scrolling down to the next section.
5. Under “Signed Request File”, select the “choose File” button and upload the signed request document that was emailed to you in section 1.
6. Click “Submit”, and you will then be automatically redirected to the payment webpage (Ark Gov Pay) if a payment is required.

You will receive a confirmation email verifying submission of your request and completed payment.

Registry check results are emailed via encrypted email to the entity identified in the ‘Results Should Be Released To’ section of the form submission request. Results will be sent in the mail to the address on the form. NOTE: Arkansas sends the results by email within 7-10 business days.

When/If the applicant or provider receives the results of the Child Abuse and Neglect History Check, they must send the result to the CCBC Unit by either:

Follow the steps below to request an Abuse and Neglect History Check and Criminal History Check for California:

If California changes its laws to allow for background checks for Child Care purposes, the procedures and steps in this guide will be updated accordingly. The CCBC Unit reviews this state's requisites regularly.

There is a $30 fee to process the CO Child Abuse and Neglect History Check (CAN). You must follow the instructions for payment options. NOTE: You cannot access the online submission process on mobile devices.

Abuse and Neglect History Check

The Commonwealth of the Northern Mariana Islands (CNMI) does not have a Central Registry of Abuse and Neglect records.

Follow the steps below to meet the requirements of an Abuse and Neglect History Check from Commonwealth of Northern Mariana Islands:

The Child Care Applicant will:

1. The applicant will click on the link to complete the (CNMI CAN) Self-Certification form. NOTE: Your actual signature is required.
2. Upon completion of the form, send the results directly to the CCBC Unit by either:

Criminal History Check

The Commonwealth of Northern Mariana Islands performs a name-based Criminal History Check for childcare purposes.

Follow the steps below to request a Criminal History Check from the Commonwealth of Northern Mariana Islands:

1. Submit a written request for a criminal history check to the Commissioner of the CNMI Department of Public Safety
2. State the purpose is to work in a licensed child care facility in the State of Michigan
3. In your letter include your name, all alias names, date of birth, full social security number, signature and date
4. Provide both your email address and physical address for the return of results
5. Mail or fax your request to:

Follow the steps below to request an Abuse and Neglect History Check from Connecticut:

1. Click on the (CT CAN) Child Abuse & Neglect form.
2. Fill in the fields of this DCF-3031 form. Do not leave any blank spaces. If something does not apply, enter “N/A."
3. Check the box "Day Care" Agency:  State of Michigan-MiLEAP-CCBC Attn: Child Care Background Check Unit, PO Box 30315, Lansing, MI 48909-7815.
4. Print the request form, sign, and date it.
5. Send the completed CT CAN form to the CCBC unit by one of the following options:
   • Email the completed form as a PDF attachment (preferred) with the applicant's name in the Subject line to: MiLEAP-CCBC@michigan.gov
   • Fax it with “MiLEAP/CCBC” on the cover sheet to 517-763-0233.

The CCBC Unit Analyst will complete the following:

1. Once the completed CT CAN form is received from the applicant, save it to the OOS folder.
2. Go to the CT CAN link and follow the instructions to request the CT CAN clearance 

The CCBC Unit should receive the results within two weeks via encrypted email.  However, if the applicant or provider receives the results of the Child Abuse and Neglect History Check, they must send the results to the CCBC Unit by either:

Delaware has an online portal and charges a $14 fee for the Child Abuse and Neglect History Check.

Follow the steps below to request an Abuse and Neglect History Check from Delaware:

• After DE sends you a Child Protection Registry Notification, log in to the portal here: (DE CAN) Child Abuse & Neglect
• Click on the Completed Requests tab
• In the CPR Letter column, select Click here to download to view and save the results letter to your computer

Delaware should send the results to the CCBC Unit within several weeks. However, if the applicant or provider receives the results of the Child Abuse and Neglect Check, they must send the results to the CCBC Unit by either:

Follow the steps below to request an Abuse and Neglect History Check from the District of Columbia:

1. Click on the (DC CAN) Child Abuse & Neglect link. This will take you to D.C.’s website for completing the Child Abuse and Neglect Registry. 
2. Follow the instructions: "To submit a CPR check application".
3. You will also find a tipsheet and tutorial with submission instructions in the link above.
4. Upload a color copy of a government-issued photo ID as well.

   The CCBC Unit should receive the results within 14 days. However, if the applicant or provider receives the results of the Child Abuse and Neglect History Check, they must send the results to the CCBC Unit by either:

Criminal History Check

There is a $7 money order fee for the Criminal History Check.

Follow the steps below to request a Criminal History Check from the District of Columbia:

To Request a Local Background Check:

1. Write a notarized letter requesting a background check and mail to: Metropolitan Police Department, Criminal History Section, 441 4th Street, NW, Room 550 South, Washington, DC 20001.
2. The letter must be notarized and include the following:
   • Full Name
   • Date of Birth
   • Social Security Number
   • Phone Number
   • Email Address
   • A copy of your photo ID
   • Self-addressed Stamped Envelope
   • Obtain a $7 Money Order payable to the DC Treasurer (Checks Not Accepted).

The District of Columbia sends the results to the applicant in the Self-Addressed Stamped Envelope in approximately 6 weeks.

When/If the applicant or provider receives the results of the Criminal History Check, they must send the results to the CCBC Unit by either:

Follow the steps below to request an Abuse and Neglect History Check from Florida:

2. The applicant must complete: To Be Completed by the Applicant.
3. Physically sign and date the form and return it to the child care provider.
4. The child care provider must complete: To Be Completed by the Requesting Agency
5. The child care provider must email the completed form and a copy of their facility license to hqw.cwr.employment.requests@myflfamilies.com

When the provider receives the results, send the results to the CCBC Unit by either:

The CCBC Unit does not require a separate Criminal History Check from Florida due to this state’s participation in the National Fingerprint File (NFF). No further action is needed.

NOTE: The CCBC Unit Analyst will obtain this clearance on behalf of the applicant.  No further action is required of the applicant or provider.

The CCBC Unit Analyst will complete the following:

1. Write a letter on the MiLEAP OOS Letterhead to GA requesting a Child Abuse and Neglect History Check.
2. Save as a PDF attachment.
3. Submit the Child Abuse and Neglect History Check on-line by going to the (GA CAN) IONS website.
4. Select Option 3, A state or government agency of any other state for the purposes of licensing and/or employing.
5. Georgia will send a confirmation email.
6. Upon receipt of the results, the CCBC Analyst will save the findings.

Results are typically received by the CCBC Unit in one week.

1. Submit a NOTARIZED letter of authorization for release of information with a self-addressed, stamped envelope to the following address:

   • Complete name, including aliases/nicknames and other names used
   • Full Social Security number
   • Date of Birth (must be 18 years or older)
   • A copy of a valid U.S. Federal/State Government issued photo ID; such as a driver's license or U.S. passport.
   • Payment of $15 in the form of U.S. Money Order or U.S. Cashier's Check payable to the Treasurer of Guam.
   • Your email address
   • Self-Addressed, stamped envelope

Illinois processes name-based Criminal History Checks through an online portal and charges a $16 fee. The Illinois criminal history process is a three-step process. You must first Get a Digital ID from Illinois. You will then use that Digital ID to Register with IL Chirp and lastly Request a Criminal History Check.

Criminal History Check

Indiana uses the same request form to process Criminal History Checks and Abuse and Neglect Checks for childcare purposes. Indiana requires the CCBC Unit to submit the request and returns results to the CCBC Unit by an encrypted email.

You only need to complete one IN Consent to Release Information form for both of your Indiana checks. Indiana processes both background checks using the same request form and sends both sets of results to the CCBC Unit.

If the results of the Indiana criminal history check are returned as inconclusive, follow the instructions:

• A Criminal Record Review Challenge will be required. If you are able to appear for an in person appointment in the state of Indiana, please contact IDEMIA at 1-877-472-6917 for a Criminal Record Review Challenge, if not go directly to https://in.state.identogo.com/ and follow the steps listed.

1. Click Schedule New Appointment
2. Click Don’t know your Service Code
3. Choose Criminal Record Review/Challenge from the agency name list and click Go
4. Agree to the disclaimer and click Go
5. Click Pay for Ink Card Submission
6. Click continue
7. On the next screen, select yes. A pop-up notification will appear, click continue
8. Complete the Application Information, click Go when complete
9. Once you have verified all information on the application is correct, click on Go
10. The fee for an Ink Cark Submission is $20.50. Select your payment method and click go.

Once you have completed the payment option, you will receive an email notification from IdentoGO. NOTE: Fingerprints must be submitted on standard FBI applicant cards (FD-258).

• Applicants must have their fingerprints taken by a local law enforcement agency or another provider that offers fingerprinting services. The fingerprint cards can be completed using either traditional ink-rolled prints or electronically captured prints that are then printed onto the card.
• Applicants must ensure that all required details are filled out on the fingerprint card. This includes your full name, date of birth, and address. If you paid in advance, be sure to add the payment confirmation number you received after completing your card payment.
• Once you have completed your fingerprint card, include a copy of your payment confirmation and mail the card to:

Kentucky provides background checks online through a two-step process and charges a $10 fee.  You must first Create a Kentucky Online Gateway Account. You then log into your account and select the CAN Payment and Verification to submit a request for an Abuse and Neglect History Check.

1. Go to KY Online Gateway (KOG) and click on the hyperlink at the bottom of the page 1 for the Kentucky Online Gateway and create a new account.

Kentucky processes name-based Criminal History Checks for childcare purposes and charges a $20 fee. Complete the request form and send it with payment to Kentucky by mail.

Follow the steps below to request a Criminal History Check from Kentucky:

2. Top of the page enter:

5. A $20 check or money order made out to Kentucky State Treasurer is required with the request.

The applicant must supply two self-addressed, envelopes with postage for this request: one addressed to themselves, and the other addressed to the requesting agency.                                                                                                                                                            
7. YOU MUST MAIL ALL ITEMS TO KENTUCKY STATE POLICE; THE ADDRESS IS ON THE FORM.

Kentucky sends the results to the CCBC Unit by mail within two weeks.

Abuse and Neglect History Check

Louisiana processes Abuse and Neglect History Checks for childcare purposes through an online portal and requires the child care provider to submit the request and pay the $25 fee. Please follow-up with the child care provider to complete this check and move on to obtain the Criminal History check.

Steps for the Child Care Provider to Request a Clearance:

1. The childcare provider must access this request online at (LA CAN) Child Abuse & Neglect and create an account.
2. An entity/agency creates a user id and requests a password by providing required information
3. Click here to view (LA CAN) Provider Required Documents to be submitted with your request to become an approved entity.
4. Once you are approved as an entity you will receive an email requesting that you change your email password in CANS. After changing your password, you are to click on the “New Registration” tab, located at the top of the home screen, and follow the prompts.
5. For Licensed Out of State Child Care Facilities and Louisiana DCFS Licensed Facilities, the (LA CAN) Form B, along with required documentation, is uploaded within the system to submit a formal request. The Request and Consent Form B is required to submit a formal request, and the (LA CAN) Form B Instructions provide assistance with completing the Request and Consent Form. Entity Type is Out of State Child Care Facilities.  
6. The entity/agency will receive an email that provides directions for obtaining the clearance results from the CANS system.

Louisiana strives to complete all clearances within 10 business days; however, the appeals process of investigative findings will likely delay responses.

Louisiana requires the submission of fingerprints to perform a Criminal History Check and charges a $31 fee.  There are three forms you must complete and send with your payment and fingerprint cards by mail to LA.

1. Complete the Authorization Form in ink.
   • Enter your information as the Individual
   • Enter N/A for agency email
   • Check the reason working with children
   • Complete the form with your vital information

4. Contact a local law enforcement agency and schedule an appointment to get fingerprinted on two (2) FD-258 cards
5. Fill in the fields on the FD-258 fingerprint card as follows:
   • Enter your name and personal identifying information, including height, weight, and place of birth
   • Signature of Person Fingerprinted – Enter your signature
   • Reason Fingerprinted – Enter “MI Child Care Licensing”
   • Purchase a money order or cashier's check for $31 made payable to Louisiana State Police
6. Send the following to the address for the Louisiana State Police:
   • Three completed forms
   • Two completed fingerprint cards

4. Check your email for a message from info@informe.org (You may need to check your Junk or Spam folder). Maine sends the results to you within two hours in most cases. If additional research is required, it may take up to two weeks.

When a record is found for the name and date of birth provided, you will receive a record that contains conviction information and pending charges less than one year old. The record provided may belong to another individual with the same or essentially similar name and date of birth, as personal identifiers, name, and date of birth, are used to conduct search.

Minnesota has an online process for obtaining the check and charges a $20 fee. You must use a credit or debit card to make the online payment.

2. The CCBC Unit will review your request and submit to MN on your behalf. Be certain you have provided all vital information and aliases.

3. Once it is submitted to MN, you will receive a notice that it has been received.
4. The CCBC Unit will receive the results, and you will receive a notice that the check is complete.
5. The CCBC Analyst will process the results as soon as possible.

Follow the steps below to request a Criminal History Check from Mississippi:

The background check processing fee of $50 will be paid online. NOTE: The "Customer Info” needs to match the billing address for the card you will be using to pay.

Once online payment is complete, visit a local sheriffs or police station or other entity to request hard card fingerprinting on a fingerprint card FD-258.  If the police/sheriff department does not have a blank hard card for you to be printed on, you can obtain a fingerprint card by contacting Brittany Orr at 601-364-5062 or email CHRCUnit@msdh.ms.gov.

If a person has resided outside the United States within the preceding five (5) years, they must sign a self-certifying statement that they are not ineligible to receive a license, be an adult member of the household, or a childcare staff member. An individual who provides or is determined to have provided false information or knowingly omits information in the self-certification statement is ineligible for that application. See MCL 722.115n(2).

2. After you have created your Individual Account, click on the Individual Log In button.

Pennsylvania processes name-based Criminal History Checks through an online portal and charges a $22 fee. 

Follow the steps below to request a Criminal History Check from Pennsylvania:

1. Go to the (PA CH) Criminal History website and select Submit a New Record Check.
2. Select the button for Individual Request.
3. Select “Employment” from the Reason for Request dropdown.

Puerto Rico is a non-responsive state for providing a Criminal History Check. However, you must complete the following steps to pass the Puerto Rico Criminal History Check Requirement:

2. Print, fill out the entire form, sign (your actual signature is required), and date the Self-Certification Statement
3. If you are an MDE applicant, list MDE as the facility.

Criminal History Check

South Dakota requires the submission of fingerprints to perform a Criminal History Check and charges a $30 fee. Complete the Authorization and Release form and send it with your fingerprints and payment to South Dakota by mail.

Follow the steps below to request a Criminal History Check from South Dakota:

5. This fingerprint card must be completed to include the applicant's name, date of birth, gender, and social security number as the minimum requirements for demographic information. Incomplete requests will be returned to the requestor.
6. The individual must take the fingerprint card to his/her local law enforcement agency (Police Department or Sheriff's Office) to be printed. There may be an additional charge by these agencies to cover the cost of fingerprinting.
7. A check or money order made payable to DCI for $30 must be sent along with the fingerprint card and completed authorization and release.
