Michigan Out of State, Territory and Country Resource Guide
This guide is only for people who are required to have a background check for connection to a childcare facility in Michigan, and who previously lived in another state, territory, or country within the last five (5) years. This guide is alphabetized, and you will find "Out of Country" accordingly.
If you were emailed a link to this guide, it is because the CCBC Unit was informed that you lived outside of Michigan within the past five (5) years.
This guide contains instructions for the following people who need Out-of-State background checks:
- Applicants for a childcare license
- Licensees and Licensee Designees
- Program Directors, childcare employees, and unsupervised volunteers
- Adult household members of a home- based childcare
- Frequent or regular visitors of a childcare facility who fall under the definition of member of household
How long do I have to complete the Out of State Process?
The CCBC Unit is required to provide an eligibility determination within 45 days of receiving the fingerprint results. However, states are not penalized for working with applicants who require Out of State background checks and are also working with the CCBC Unit to complete those checks. That does not mean that the department will allow individuals to linger in an Additional Information stage.
Individuals will be provided with 45 days from the date this guide was emailed to them to provide completed results of all their required out of state background checks.
WARNING:
• Although you have been provided with 45 days from the date we emailed the link to this guide, you must immediately begin the process of submitting the background check requests. Some states may take several weeks to provide results.
• Applicants who do not send their completed results by the date indicated on their letter will be made Ineligible due to Failure to Submit Additional Information.
• If you do not receive a link via email, the latest version of the guide can be found at: www.michigan.gov/ccbc under the Forms and Resources tab.
• If your form(s) are rejected due to updates made by the receiving state, follow the state(s) new process, and contact your CCBC Analyst.
• Do not submit Out of State information prior to receiving your Additional Information Letter. Information received prior to having had a
fingerprint result will be returned to the sender and will not be retained by the CCBC Unit.

Alabama

Minors in Child Care Operations: Alabama does not perform Abuse and Neglect History Checks for minors in childcare operations. If the person is between the ages of 14 – 17 years old, no further action is required for this check.

Criminal History Check
Alabama requires the submission of fingerprints to perform a Criminal History Check and charges a $25 fee. Complete the request form and send it with your fingerprints and payment to Alabama by mail.
Follow the steps below to request a Criminal History Check from Alabama:
- Click on the link to the (AL CH) Criminal History form (Instructions are included).
- Alabama should send the results to the CCBC Unit by mail in about four to five weeks.
- If the applicant or employer receive the results, they must be forwarded to MiLEAP-CCBC@michigan.gov.
- Please note that there are delays with mail processing, so it may take to receive and process the results.
- When/If the applicant or provider receive the results of the Criminal History Check, they must send the request form to the CCBC Unit by either:

Alaska

Abuse and Neglect History Check
Currently, there is no know fee to complete the AK Abuse and Neglect History Check.
Minors Under Age 16: Alaska does not perform Abuse and Neglect History Checks for people under the age of 16. If the person is between the ages of 14 – 15 years old, no further action is required for this check.
Follow the steps below to request an Abuse and Neglect History Check from Alaska:
- Click on the (AK CAN) Child Abuse & Neglect form.
- Complete the form
- Print out the Alaska clearance form
- Sign on the Household Member Signature line and date it
- Make a copy of your photo id and submit with the request form
- Submit the form to Alaska by either:

American Samoa performs name-based Criminal History Checks for childcare purposes and requires the CCBC Unit to submit the request.
Follow the steps below to request a Criminal History Check from American Samoa:
CCBC Unit will:

Arkansas

Follow the steps below to request an Abuse and Neglect History Check from Arkansas:
There are three stages involved with receiving results for an Arkansas (AR) Child Maltreatment Central Registry Background Check:
Stage 1 : Arkansas (AR) Child Maltreatment Central Registry Form Generator Process
- Access the (AR CAN) Child Abuse & Neglect website. After reading through all three stages and their associated steps below, click on the Central Registry Request Form Generator link on the webpage.
- Once you are on the AR Child Maltreatment Central Registry Request Form web page, complete each required field (required fields denoted by an asterisk).
- Once all required fields are completed, click ‘Submit Form’ button.
- The completed request form will be emailed to the email address you entered on the AR Child Maltreatment Central Registry Request Form web page.
- Print this form and sign the completed copy in the presence of a Notary.
Stage 2 : Arkansas (AR) Child Maltreatment Central Registry Form Upload Process
- Scan the notarized form to your computer and save it to a place you can easily locate it.
- Once you have your form saved, access the Central Registry Form Uploader to send your file to the Central Registry Team using the links below:
- Click ‘Browse’ under ‘Notarized Request File’ to search your computer and upload the notarized copy of the form. (For multi-submissions, you must upload a notarized form for each request). The notarized form should consist of a minimum of two pages and both pages must be uploaded.
NOTE: Once you have reached the step to upload your notarized form, please enter in the following information under the RESULTS SHOULD BE RELEASED TO section:

Stage 3 : Online Payment
- Select a Payment Type.
- Complete the customer information and payment information sections to pay the central registry check fee outlined below:
- For single requests: $10.00 for the child maltreatment request + a $1.00 online processing fee
- For multiple requests paid by credit card: $10.00 for each child maltreatment request + 3% of request total amount + an online processing fee of $2
- For multiple requests paid by e-Check: $10.00 for each child maltreatment request + a
- Click ‘Submit Payment.’
- You will receive a confirmation email verifying submission of your request and completed payment.
- Registry check results are emailed via encrypted email to the entity identified in the ‘Results Should Be Released To’ section of the form submission request. Results will be sent in the mail to the address on the form.
- When/If the applicant or provider receive the results of the Child Abuse and Neglect History Check, they must send the result to the CCBC Unit by either:

On-Line Submission Process (Preferred Method)
- To begin the online request application process, please click on the (CO CAN) Child Abuse & Neglect online link (Note: Google Chrome browser is preferred).
- The fee is $30 to process the CO Child Abuse and Neglect History Check (CAN). The following payment methods are accepted: credit card and e-check.
- You will need a copy of one of the following to submit your request: Driver’s License, Passport, State-Issued Identification Card, Military Identification Card, Social Security Card, or Birth Certificate.
- Compete the on-line application and submit with fee.
- Online requests will be processed within 10 days.

Paper Process for Applicants without On-Line Access (Results in a Delay of Processing) If you need to submit a paper request via United States Postal Service:

- Select Get Started and follow the prompts.
- Complete, print and then sign (ink to paper) the attached CO Individual Child Abuse and Neglect (Trails) Request.
NOTE: Only typed forms will be accepted. They will not accept a hand-written form.
- Results from the individual request are released only to the person being checked. The person being checked must be listed as the requestor.
- Once the results of the CO Individual Child Abuse and Neglect (Trails) Request are received, send the results to the CCBC Unit by either:

Delaware has an online portal and charges a $14 fee for the Child Abuse and Neglect History Check.
Follow the steps below to request an Abuse and Neglect History Check from Delaware:
- Click on the (DE CAN) Child Abuse & Neglect consent form.
- Complete all relevant sections of the form.
- Print, sign, and date the DE consent form.
NOTE: If the person is under the age of 18, a parent or guardian must sign the form.
- Scan the completed consent form and save it to a folder on your computer.
- You will need a credit card to pay the required $14 fee.
- Go to the (DE CAN) Child Abuse & Neglect registry request portal.
Note: You must use a Google Chrome or Safari web browser as this portal is not supported on Internet Explorer, Microsoft Edge, and other browsers.
- Refer to the (DE CAN) Child Abuse & Neglect Instructions while following the steps below:
Registration
- Register as an individual by following steps 1-7 in this section of the instructions
- Submit your request and upload the DE Consent Form by following steps 8-14
- NOTE: You must enter the following Agency numbers to share your results with the CCBC Unit:

District of Columbia

Abuse and Neglect History Check
The District of Columbia (D.C.) processes Abuse and Neglect History checks for childcare purposes through an online portal. Complete a request form and upload it with a copy of your ID to the D.C. portal. There is no known fee for the Child Abuse and Neglect Registry Check.
Follow the steps below to request an Abuse and Neglect History Check from the District of Columbia:
- Click on the (DC CAN) Child Abuse & Neglect link. This will take you to D.C.’s website for completing the Child Abuse and Neglect Registry.
- Follow the (DC CAN) Instructions for completing and submitting the request.
NOTE: Submit the application here for any individual getting a first-time clearance for work with your organization - a new employee, contractor, volunteer or intern or a current employee, contractor, volunteer, or intern who has a new position which requires a Child Abuse and Neglect History Check.
*Be sure to upload a color copy of a government-issued photo ID as well.
*As a courtesy, applications for new hires are processed within 14 days of receiving a correct and complete application.

Georgia allows the CCBC Unit to request the Child Abuse and Neglect History Check on-line. Therefore, no action is needed by the applicant.
The CCBC Unit Analyst will complete the following:
- Write a letter on the MiLEAP OOS Letterhead to GA requesting a Child Abuse and Neglect History Check.
- Save as a PDF attachment.
- Submit the Child Abuse and Neglect History Check on-line by going to the (GA CAN) IONS website.
- Select Option 3, A state or government agency of any other state for the purposes of licensing and/or employing.
- Georgia will send a confirmation email.
- Upon receipt of the results, the CCBC Analyst will save the findings.
Results are typically received by the CCBC Unit in less than one week.

Illinois processes name-based Criminal History Checks through an online portal and charges a $16 fee. Submit a request to Illinois in the online portal.
Follow the steps below to request a Criminal History Check from Illinois:
Name-based checks are available online through a two-step process. You must first Get a Digital ID from Illinois. You then use that Digital ID to Register with IL Chirp and Request a Criminal History Check.
To Get a Digital ID –You must first obtain a Digital ID to access the Illinois Criminal History Information Response Process (IL CHIRP) website:
- Go to the (IL CH) Criminal History CHIRP website.
- Click on the Get a Digital ID button.
- Follow the prompts
- Once you have received your reference number via email and the authorization code via USPS mail from the State of IL, complete the steps to register and activate a Digital ID below.
- Go to the (IL CH) Criminal History CHIRP website and enter the Username and Password associated with your Digital ID
- Click the Register Here button to register as a Public User (No ORI number is required):
- Accept the terms and conditions
- Provide your name and contact information
- Provide a valid email address –This will be used to log in
- Create a password.
- Select your secret question and answer.
- Follow the activation instructions sent to your email account.
Request a Criminal History Check –After you have registered, access IL CHIRP to submit a background check request:
- Go to the (IL CH) Criminal History CHIRP website and enter the Email and
Password associated with your CHIRP Login
- Hover your cursor over Create Inquiry then select Add Inquiry from the menu displayed
- Enter your credit card or eCheck information in the Payment Type section
- Enter your personal information in the Subject Information section
- Click on the Save Inquiry button
- Click on the View Cart button to submit the request and check out
- On the Welcome to Chirp webpage, hover your cursor over the Check Status tab and select My UCIA Inquiries from the menu displayed

Indiana uses the same request form to process Abuse and Neglect History Checks and Criminal History Checks for childcare purposes. Indiana requires the CCBC Unit to submit the request.
- The form applicants submit to the CCBC Unit is the (IN CH-CPS) Criminal History & Child Abuse & Neglect form, 53323.
- If you already printed, completed, and returned the IN Consent to Release Information state form 53323 for the Child Abuse and Neglect History, another form does not need to be submitted.
- Once the CCBC analyst receives the results, the unit will document and process the background check component.

Louisiana processes Abuse and Neglect History Checks for childcare purposes through an online portal and charges a $25 fee. Louisiana requires the childcare facility to submit the request in the portal and pay the $25 fee.
NOTE:
Minors: Louisiana does not perform Abuse and Neglect History Checks for minors. If the person is between the ages of 14 – 17 years old, no further action is required for this check.
Follow the steps below to initiate an Abuse and Neglect History Check from Louisiana:
Louisiana requires the childcare facility to register in their background check portal and submit the request. Please follow-up with the childcare provider to complete this check. They require a fee of $25 per request.
The Child Care Provider:
Steps for Child Care Provider to Request a Clearance:
- The childcare provider must access this request online at (LA CAN) Child Abuse & Neglect
and create an account.
- An entity/agency creates a user id and requests a password by providing required information; Click here to view a (LA CAN) Provider Required Documents to be submitted with your request to become an approved entity.
- Once you are approved as an entity you will receive an email requesting that you change your email password in CANS. After changing your password, you are to click on the “New Registration” tab, located at the top of the home screen, and follow the prompts.
2. For Licensed Out of State Child Care Facilities, and Louisiana DCFS Licensed Facilities, the (LA CAN) Form B along with required documentation, is uploaded within the system to submit a formal request. The Request and Consent Form B is required to submit a formal request, and the (LA CAN) Form B Instructions provide assistance with completing the Request and Consent Form. Entity Type is Out of State Child Care Facilities.
3. Submit the non-refundable fee of $25.00, per request.
- The entity/agency will receive an email that provides directions for obtaining the clearance results from the CANS system.
- Louisiana strives to complete all clearances within 10 business days. However, the appeals process of investigative findings will likely delay responses from DCFS.
- Upon receipt of the results, submit the results to the CCBC Unit by one of the following:

- The CCBC analyst will access the MS CAN link to begin the DocuSign process for the MS CAN clearance request and completes steps 2-6 below:
- The CCBC analyst will enter in their name and email address under the “Requestor Information” section.
- The CCBC analyst will enter in the applicant’s name and email address under the “Applicant Information” section.
- Once the CCBC analyst completes steps 2 & 3, press the “Begin Signing” button.
- The CCBC analyst will complete the required fields for the person requesting the check.

- The CCBC analyst will complete the required steps to electronically sign the form and press the “Finish” button.

Once online payment is complete, visit a local sheriffs or police station or other entity to request hard card fingerprinting on a fingerprint card FD-258 . (If the police/sheriff department does not have a blank hard card for you to be printed on, you can obtain a fingerprint card by contacting Julie Henderson at 601-364-5059 or email CHRCUnit@msdh.ms.gov). Under PA 116, as Amended, if a person has resided outside the United States within the preceding five (5) years, they must provide results of equivalent clearances to the Child Abuse and Neglect History Check, Criminal History Check and Sex Offender Registry Check from each country they have resided. If the country does not have the equivalent clearance(s), the individual must sign a self- certifying statement that he or she is not ineligible to receive a license, be an adult member of the household, or a childcare staff member. An individual who provides or is determined to have provided false information or knowingly omits information in the self-certification statement is Ineligible for that application. See MCL 722.115n(2).

South Dakota requires the request to be submitted by mail. Complete the request form and send it to South Dakota by mail.
Follow the steps below to request an Abuse and Neglect History Check from South Dakota:
- Open the (SD CAN) Child Abuse & Neglect form. The reason for the request is: Licensed child care employment in the State of MI
- Print and complete the SD CAN form.
- Sign and date the request form in front of a Notary
- Have the notary sign and seal the request form

Criminal History Check
South Dakota requires the submission of fingerprints to perform a Criminal History Check and charges a $26.75 fee. Complete the request form and send it with your fingerprints and payment to South Dakota by mail.
Follow the steps below to request a Criminal History Check from South Dakota:
- Click on (SD CH) Criminal History form, which is called the Authorization and Release.
- Print and complete this form and release to: State of Michigan-MiLEAP-CCBC in each paragraph. There are two paragraphs. Mail Response to: State of Michigan-MiLEAP-CCBC, P. O. Box 30315, Lansing, MI 48909
- A state applicant fingerprint card must be obtained from the DCI. All necessary supplies are available through the DCI Identification Section at no cost by calling (605) 773-3331.
- This fingerprint card must be completed to include the applicant's name, date of birth, gender, and social security number as the minimum requirements for demographic information. Incomplete requests will be returned to requestor.
- The Authorization and Release form printed on the back of the must be filled out and signed. Be sure to include the correct address to which the response should be sent.
- The fingerprint card must also include a fully rolled set of fingerprints per SDCL 23-5-12.
- The individual must take the fingerprint card to his/her local law enforcement agency (Police Department or Sheriff's Office) to be printed. There may be an additional charge by these agencies to cover the cost of fingerprinting.
- A check or money order made payable to DCI for $26.75 must be sent along with the fingerprint card and signed authorization. Do not send cash.
- Upon receipt of all necessary information, fingerprints, signed authorization and fee, the DCI will conduct a state search and supply a copy of any criminal history that is found, or a letter stating that there is no criminal history. These results do not include federal charges, juvenile offenses (unless charged as an adult), sealed records, minor traffic violations or out of state information. Fingerprint cards will be destroyed and will not be returned with the response.
- When/If the applicant or provider receive the results of the Criminal History Check, they must send the request form to the CCBC Unit by either:

Tennessee1. Obtain the Child Abuse and Neglect History Check for this state as described below.
Tennessee allows the CCBC Unit to submit the request for childcare purposes. Therefore, the CCBC Unit will take steps to submit the request. No action is needed by the applicant.
Follow the steps below to request an Abuse and Neglect History Check from Tennessee:
The CCBC Analyst will:
- Complete the TN CAN form with applicant's previous address and dates of residence
- Fill in a MiLEAP OOS Letterhead requesting the information for the Tennessee CAN. This will be sent in the email that contains the (TN CAN) Child Abuse & Neglect form.
- Send email to: CPS.Background.Check@tn.gov along with the following:
- (TN CAN) Child Abuse & Neglect form in WORD format
- MiLEAP OOS Letter
- Tennessee sends the results to the CCBC Unit by email within seven to ten business days.

NOTE: The CCBC Analyst forwards the request to Texas for the Child Abuse and Neglect History, because results take approximately 95-100 days to receive. If the CCBC Unit submits the request, we have the ability to invoke Due Diligence and make applicant Eligible when we can verify that the request was submitted within the necessary time frame.

- Complete applicant information and sign and date it in front of a Notary
- Have the notary sign and stamp the request form.
- If you choose to email the form you must use our email address for the return of the check:

Virginia processes name-based Criminal History Checks through an online portal. You will need to submit a request and payment to Virginia in the online portal. The fee is $15 for the Criminal History Check and an additional $15 if you notarize the form through the online NotaryCam option offered by Virginia.
Follow the steps below to request a Criminal History Check from Virginia:

West Virginia requires the request to be submitted by mail. Complete the request form and send it to West Virginia by mail.
Minors: West Virginia does not perform Abuse and Neglect History Checks for minors. If the person is between the ages of 14 – 17 years old, no further action is required for this check.
Follow the steps below to request an Abuse and Neglect History Check from West Virginia:

Wyoming currently has a $10 fee plus processing fee to process the Abuse and Neglect History Check. They accept Credit and Debit cards for VISA, Mastercard, DISCOVER, AMERICAN EXPRESS, as well have Digital Wallets.
Follow the steps below to request an Abuse and Neglect History Check from Wyoming:
- Click on the link to the (WY CAN) Child Abuse & Neglect website.
- Follow the (WY CAN) Portal Instructions to Submit a New Central Registry Screen.
- Pay fee and Submit request.
- The requestor will receive an email with a link to retrieve the results when ready.
NOTE: Pursuant to state statute and agency rules, all results reporting that an applicant is listed on the Central Registry will be sent by hard copy through postal service mail.
- When/If the applicant or provider receive the results of the Child Abuse and Neglect History Check, they must send the result to the CCBC Unit by either:
