10.07.03: Correcting the supplemental TDP agreement

In the event a Supplemental TDP Agreement (R0654C) form is submitted with an incorrect date on line 6, you are allowed to make a correction to the form.

How to correct the form:

  1. Correct the date on line 6.
  2. Initial and date your correction.
  3. Write "AMENDED" boldly across the top of the form. 
  4. Fax a paper copy of the agreement to ORS at 517-284-4416. You do not need to submit the supplemental agreement to ORS on the TDP agreement file.

An amended supplemental agreement will not be processed if it attempts to lower the scheduled deduction amount below the amount already on file.
