State Agency Confidential Information Requests

Under the provisions of a data sharing agreement (DSA) or when other legal authorities require the release of information, Department of Treasury may provide as filed tax return information to State Agencies upon request. State Agency requests should be submitted through the Michigan Disclosure Request System.

    If a state agency needs to establish a DSA with Treasury, the first step is completing the DSA questionnaire. Once a DSA is in place, an authorized representative may begin requesting information. As part of Treasury’s DSAs, state agencies are responsible for completing annual compliance requirements.

  • DSA Questionnaire

    The Pre Agreement Questionnaire gathers essential details from agencies about what authorities grant them access to Michigan Treasury tax information, how they will use it, and who will have access to it. Once submitted, Department of Treasury will review and determine if a DSA is authorized.

  • Confidentiality Agreement

    Form 4062, is an employee confidentiality agreement required for all authorized personnel who access confidential state tax information. Any employee must complete the agreement prior to having access to treasury data.

  • Verification of Confidentiality Compliance

    State agencies with access to Treasury data are required to submit this form annually, confirming employees with access have completed their compliance requirements.
