Disclosure Requests, Forms, and Information

Disclosure Requests

Disclosure requests are formal requests for confidential or certified information held by Treasury. These requests often come from taxpayers, authorized third-party vendors, courts, law enforcement, and government agencies. Examples include income verification, tax status letters, subpoenas and certified documents for legal proceedings. Until now, these requests were processed manually through mail or fax.

As of January 9th, 2026, the preferred method for submitting a request is through our new modernized system:
Michigan Disclosure Request System (MiDRS)

MiDRS enhances the customer experience with real-time status tracking and enables faster turnaround times. The system also allows for online payment options (when applicable) and electronic, secure delivery of documents.

If you are unable to submit via MiDRS, you may submit the applicable forms and payments by mail to:

Office of Privacy and Security, Disclosure Unit
Michigan Department of Treasury
430 W. Allegan St.
Lansing, MI 48922
