The Office of the Taxpayer Advocate helps taxpayers and tax professionals that have been unable to resolve a state tax issue. Only use the following form if you have already tried to resolve your issue using Treasury’s normal channels and still need assistance. Please provide detailed information and complete each field marked “required.”

Please note: Fields marked as “required” must be completed in order for the request to successfully be submitted and received by the Office of the Taxpayer Advocate.
