- Step 1: Complete the MiDEAL Customer Choice Pricing Request Form.
- Step 2: Email the completed form to both vendors listed below and copy the State of Michigan consultant, Landmarkec@comcast.net.
- Step 3: The winning vendor will contact you shortly after submitting the form. At that time the vendor will provide a Schedule A document outlining the details of the transaction.
- Step 4: Sign and return the Schedule A document provided by the winning vendor.
