Oil and gas forms

EGLE GRMD has implemented electronic document management practices and strives to operate in a paperless environment. Accordingly, forms and reporting can now be electronically completed and submitted. Operators must ensure that all forms and reports are accurately completed and submitted on time in accordance with all applicable regulations. GRMD reviews the submitted forms and reports for compliance and when necessary, will notify operators of required revisions.

Although many of the forms may be submitted by email to a specified email address as indicated below, please know that most of the required injection and production reporting is now accomplished exclusively through the Michigan's State Tracking and Reporting System (MiSTAR). The use of MiSTAR for reporting as specified below is now mandatory for operators in Michigan. GRMD has created detailed guides for using MiSTAR which are available for downloading from our website.

Permit Applications utilize the Electronic Form Permit Application (E-form) system. The E-form system collects information that is from the application cover page form (EQP 7200-1). All log files shall be submitted in Log ASCII Standard (LAS) and Tag Image File (TIF) format. PDF files and paper copies may be accepted with prior approval by EGLE, GRMD. Files shall be named using the well permit number, well name, and log type. All files are to be submitted as single page TIF and/or single page PDF. logs are due within 30 days of conducting the logging run.
