Information and Application to Receive UI Data

Unemployment Insurance Agency (UIA) information is confidential, but may be shared, by request, under specific circumstances.  Once your request is reviewed, you will receive notice as to whether your request was approved or denied. Please see Fact Sheet 170, Unemployment Insurance Agency Data Requests for more information on requesting data.

The four types of information requests made to the UIA that can be provided to eligible requestors include:

• requests for individual records;
• requests by Public Officials, Michigan Works! Agencies and Educational Institutions;
• requests under the Freedom of Information Act (FOIA); and
• requests for data that may be provided from partner agencies such as Workforce Development Agency (WD) and Labor Market Information (LMI). 

Any person may request their own records or authorize the UIA to release his/her records to a third-party. To request personal data, submit a completed Form UIA 6102, Authorization to Release Confidential Information.   

Public officials, eligible educational institutions, a Michigan Works! Agency, an agency of this state that is acting as a contractor or agent of a public official that is conducting research that assists the public official in carrying out the duties of the office may request data using the following forms:

Form UIA 6439 – Public Official, Educational Institution, or Michigan Works! Agency Request for Information

Form 6467 -  Michigan community College Request Form for Unemployment Insurance Data – SSN (use this form if you are making the request on behalf of a Michigan Community College and can provide individual Social Security Numbers related to your request)

Form 6468 -  Michigan Community College Request Form for Unemployment Insurance Data (use this form if you are making the request on behalf of a Michigan Community College and cannot provide individual Social Security Numbers related to your request).
