Eye doctors are often the first professionals that people consult when they experience vision loss. After doing all that you can do professionally, you can do even more -- by referring your eligible low-vision patients to the (Michigan) Bureau of Services for Blind Persons for services.

Eligibility
Your patients must meet certain criteria to become a BSBP customer.
- BSBP Eye Report Form (PDF) Use this form to refer eligible blind and visually impaired patients age 14 and over to the Bureau of Services for Blind Persons.
- BSBP Eye Report Form (Accessible Format) (This Microsoft Word file is accessible with JAWS screen-reader software.)
