Reporting Fatalities and Injuries
Report a Work-related Fatality
It is required to report fatalities within 8 hours after the death of any employee from a work-related incident by phone to MIOSHA.
To report work-related fatalities/catastrophes, call MIOSHA’s toll-free number: 800-858-0397.
Report a Work-related Severe Injury/Illness
The Employee Injury/Illness Incident Report Form is to be used by Michigan employers to report work-related employee incidents that result in the loss of an eye, an amputation, or inpatient hospitalization within 24 hours of the incident.
The above form is also to be used by Michigan employers filing a power press injury report as required General Industry Standard Part 24, Mechanical Power Processes. An employer must file the report within 30 days of the occurrence of all point of operation injuries or injuries within the confines of the die to operators or other employees.
