Report a Work-related Fatality

Within eight hours after the death of any employee from a work-related incident, you must report the fatality by phone to MIOSHA.

Report a Work-related Severe Injury/Illness

The Employee Injury/Illness Incident Report Form is to be used by Michigan employers to report work-related employee incidents that result in the loss of an eye, an amputation, or inpatient hospitalization within 24 hours of the incident.

Required fields are indicated by *. Please make sure all asterisked fields are completed. If you have questions about filling out this form, please call (844) 464-6742. Once you submit the form, you will receive an email response from MIOSHA with your report number.

As an alternative, employers can provide the information through the MIOSHA Severe Injury Report Line at 844-464-6742, Monday through Friday, 8:00 a.m. to 5:00 p.m. or leave a message after hours.
