Laboratory and Customer Agreement for Simplified Reporting

All Michigan State Police (MSP) Biometrics and Identification Division Combined DNA Index System (CODIS) laboratory reports will be issued in a simplified format. Each report will contain a title, the name and address of the laboratory, the location of the testing activities, and case information including:

• Name of the customer
• Submitting agency’s case number
• MSP laboratory number
• Date the report was issued
• Identification of methods used
• Analyst responsible for authorizing the report

Additional information is available in the case file and will be made available to the submitting agency upon request. The information included in the case file includes, but is not limited to:

• Contact information for the customer
• Date the sample was received by the CODIS laboratory
• Dates of laboratory testing activities
• Approved additions, or deviations to standard protocols

Uncertainty of measurement calculations, sampling plans, opinion statement, and interpretation statements are not applicable for CODIS laboratory reports.
