Hazardous waste program forms and license applications
This page provides the forms, instructions, and payment information for facilities regulated under Michigan’s Part 111 (Hazardous Waste Management) or Part 121 (Liquid Industrial By-Products) of the Natural Resources and Environmental Protection Act, 1994 PA 451, as amended, and the federal Resource Conservation and Recovery Act (RCRA).
Site identification (EQP5150 Form)
Overview
Certain facilities must submit the Michigan Site Identification Form (EQP5150 – Revised 6/2025) to:
- Obtain a new Site Identification Number (initial notification)
- Submit a Part A permit component
- File a Hazardous Waste Biennial Report
- Submit subsequent notifications
- Fulfill the Annual User Fee Charge requirement for some hazardous waste generators
When you need a site ID
Submit the form when:
- Your facility generates, transports, treats, stores, or disposes of hazardous waste
- You handle liquid industrial by-products
- Ownership or legal entity of an existing site changes
- Your site has never been issued a Site ID
Check if your site already exists
Before completing the form:
- Search the EGLE database to see if the site location already exists (even if under a different name).
- If no record exists, download and complete the EQP5150 form (Revised June 2025).
- A $50 fee applies for a new Site ID.

If the site exists under prior ownership, download and complete the EQP5150 form using the existing Site ID number.
- A $50 fee applies for new ownership of a registered site.

The EQP5150 form replaces several older EPA forms:
- EPA Notification of Regulated Waste Activity (Form 8700-12)
- EPA Hazardous Waste Permit Part A (Form 8700-23)
- Michigan Notification of Regulated Waste Activity
- EPA Identification and Certification Form (Form 8700-13A/B)
The EQP5150 form is also used with the Michigan Hazardous Waste Operating License Application (EQP5111).
How to pay the $50 user charge fee
A $50 user charge must be submitted for:
- New hazardous waste generators, transporters, or treatment/storage/disposal facilities
- Liquid industrial by-products transporters
EGLE will not issue a site ID number until payment is received and cleared.
Option 1 – online payment
- Pay by credit card (MasterCard, VISA, or Discover) through The Pay Place.
- Include your payment receipt when submitting your EQP5150 form.

Write “Site ID user charge” on the check to ensure proper credit.
When the fee applies
You must pay the $50 fee when:
- A Site ID number has never been assigned to the physical location.

A new owner or legal entity takes over a site (for example):
- The business is sold or transferred
- The entity type changes (e.g., becomes an LLC, LLP, or Inc.)
- The business owners or principals change
Emergency Site Identification Numbers
An Emergency Site ID is issued when unexpected incidents prevent waste from being safely accumulated onsite (e.g., roadside spills, fire hazards, unstable conditions).
- Valid only until the immediate threat is resolved
- Issued conditionally upon $50 user charge payment
- Site-specific and nontransferable
How to request an Emergency Site ID
During business hours:
Call EGLE’s Permitting and Program Support Unit at 517-582-4400
After hours, weekends, or holidays:
Call the Pollution Emergency Alerting System (PEAS) at 800-292-4706
After reporting:
- You’ll receive a copy of Form EQP5126 generated from your phone report.
- Complete any missing fields (e.g., Federal ID Number).
- Mail the completed form and $50 check payable to “State of Michigan” to:

Hazardous waste operating license applications
Facilities that treat, store, or dispose of hazardous waste must submit an Operating License Application.

The EQP5111 + EQP5150 forms together replace the federal RCRA Part A Permit Application.
