RIDE Submittal Types & Guidance

The Remediation and Redevelopment Division (RRD) requires that submittals use the appropriate form. Many of these forms are now only available electronically through RIDE. Submittals provided through regular mail, email, and other means without the required electronic submittal form in RIDE may not comply with the applicable regulatory requirements to be accepted by the department.

Several forms have been modified to support EGLE’s Risk Based Corrective Action (MIRBCA) program and other Part 201 program requirements.  Details on the requirements associated with each type of form are provided below.

Part 201 Submittals through RIDE

The following submittals for the Part 201 program must be submitted directly through RIDE. If the subject facility is not included on the current Inventory of Facilities, the ‘Facility Not Found’ feature can be used to provide the information. The pdf of the EQP form should not be included as part of the RIDE Attachment; the electronic submittal replaces the use of the pdf form.

Part 201 - Supplemental Forms

The supplemental forms for the Part 201 program are listed below. Generally, these are provided as an attachment to the applicable RIDE submittal (e.g., Response Activity Plan, NFA). If the form is being provided without an accompanying submittal, you may use the Non-Form option for either an existing facility or the ‘Facility Not Found’ feature in RIDE.

EQP4483 – Request for Mixing Zone Based GSI Criteria - NOTE: This form should only be used to accompany a Response Activity Plan submittal for 201 facilities or a Supplemental Information Report for 213 facilities.

EQP4029 – Consultant Affidavit for NFA Report

EQP4031 – Person Submitting Affidavit for NFA Report

EQP4420 – Response Activity Plan Review Panel - NOTE: This form must be provided via regular US Mail and include the accompanying payment.

EQP4468 – RIDE Risk Condition Classification - NOTE: This is an optional form that can be provided with any Part 201 submittal

 

*NOTE: Form “EQP4380 - Request for EGLE Review of Response Activity Plan to Comply with 7a(1)(b)” has been rescinded. Users seeking review of this type of Response Activity should use form EQP4028 – RAP and select ‘To Comply with Section 20107a(1)(b)’ as the type of response activity plan being submitted.

Part 213 Submittals through RIDE

The following submittals for the Part 213 program must be submitted directly through RIDE. If the subject facility is not included on the current Inventory of Facilities, the ‘Facility Not Found’ feature can be used to provide the information. The pdf of the EQP form should not be included as part of the RIDE Attachment; the electronic submittal replaces the use of the pdf form.

The supplemental forms for the Part 213 program are listed below. Generally, these are provided as an attachment to the applicable RIDE submittal (e.g., Final Assessment Report, Closure Report). If the form is being provided without an accompanying submittal, you may use the Non-Form option for an existing facility.

Non-Form Examples

If your submittal does not fit under the listed submittal types, please use a Non-Form. Examples of Non-Form submittals include but are not limited to:

  • Addendum to previously submitted report
  • Environmental Expanded Triage Reports
  • Annual Operation & Maintenance Report
  • Bi-Annual Operations & Maintenance Report
  • Extension Requests, Miscellaneous Correspondence
  • Well Abandonment Reports
  • Restrictive Covenant
  • Draft submissions under legal agreements
  • Final submissions that do not have an associated EQP Form or submittal type available in RIDE (e.g. QAPP, progress report, FAM five-year cost estimate, IRA Report, RI Report, and Evaluation Report)
