Step 3: submit your request
The last option to select is the 'Preferred Method to Receive Records' - you may select electronic, inspect on-site, or by regular mail.
If you need to add an attachment to your request, you may do so by uploading them at the end of the form by selecting 'Attach'.
When you have completed the form, select 'Submit' to finish your request. Your request is uploaded to the system and staff are notified of your request.
