Sample reporting

All lead and copper samples must be reported to EGLE. Water supplies must use one of the following forms to report the results and all associated tiering criteria and sample location information.

    Monitoring paperwork should be uploaded to MiEHDWIS as a Lead and Copper Monitoring and Report Form activity.

Report form

All sample results and the report form are due to EGLE within 10 days after the end of the monitoring period. The signed consumer notice certification and one sample copy of a consumer notice must be submitted to EGLE within 3 months of the end of the monitoring period. It is recommended, when possible, to submit the report form, consumer notice certification, and one sample copy of a consumer notice all at once, within 10 days after the end of the monitoring period.

The consumer notice template is due to EGLE within 30 days after the end of the monitoring period. The consumer notice can also be used for non-compliance samples.

The consumer notice is due to the resident or persons served by the tested taps within 30 days of knowing the result. The consumer notice can also be used for non-compliance samples.

Michigan water supplies must sample for lead and copper using a risk-based tiering criteria. This requires maintaining a site sampling plan that meets the sampling requirements.
