Oil and gas forms
EGLE’s Geologic Resources Management Division (GRMD) now uses electronic document management and works toward a paperless process. Most forms and reports can be completed and submitted online. Operators are responsible for making sure all forms and reports are filled out correctly and submitted on time as required by regulations. GRMD reviews all submissions for compliance and will contact operators if revisions are needed.
Many forms can still be emailed to the addresses listed below. However, most required injection and production reports must now be submitted only through Michigan’s State Tracking and Reporting System (MiSTAR). Using MiSTAR for the reporting listed below is mandatory for all operators in Michigan. GRMD has created step-by-step MiSTAR guides, which are available on the MiSTAR webpage.
Permit & bonding forms

Permit applications must be submitted through the Electronic Form Permit Application (E-form) system. This system collects information from the application cover page form (EQP 7200-1).
All other submittals for permitting and conformance bonding should be submitted to EGLE-GRMD-PermitApplications@Michigan.gov.

Important: If a form is required to be submitted through MiSTAR Production or MiSTAR Injection, you must download the form or template directly from the MiSTAR system before submitting it. Instructions and guides for using MiSTAR are available on the MiSTAR webpage.

All log files shall be submitted in Log ASCII Standard (LAS) and Tag Image File (TIF) format. PDF files and paper copies may be accepted with prior approval by EGLE, GRMD. Files shall be named using the well permit number, well name, and log type. All files are to be submitted as single page TIF and/or single page PDF. logs are due within 30 days of conducting the logging run.
District approval & reporting forms
The district approval and reporting forms must be sent to the GRMD area geologist and to the appropriate district office email address for approval.
