Closure Report (CR) submittal instructions

A Closure Report is submitted pursuant to Part 213, Section 324.21312a of the Leaking Underground Storage Tanks, of the Natural Resources and Environmental Protection Act, 1994 PA 451, as amended, to demonstrate that environmental response activities at a contaminated site have been completed.

EGLE requires that this submittal type be provided using the Remediation Information Data Exchange (RIDE) system. Specific information on uploading submittals using RIDE is on the Electronic Submittals guidance page. If you are already a RIDE User, navigate to MiLogin for Business to access RIDE.

This submittal requires both primary certification (usually a consultant) and secondary certification (the regulated entity). Certifications act as the electronic signatures for submittals using RIDE.  For more information, please visit the secondary certification guidance page.

The following elements must be provided for the submittal to be accepted by EGLE:

• Secondary Certification by the Regulated Entity is required. If the consultant is performing Secondary Certification on behalf of the Regulated Entity, then EQP4032 must be completed and placed as the first pages of the Attachment.
• A “Revised” Closure Report may be submitted by checking the box only if a previous Closure Report was entered into RIDE and was denied or determined to have insufficient information. If the Closure Report being revised predated RIDE, only the ‘New’ option will be available but the same statutory requirements for review will apply.
• The MIRBCA forms are required. More information on the MIRBCA forms can be found on EGLE’s Leaking Underground Storage Tank webpage.
• Certificates of Insurance are required pursuant to 324.213(12)(a)(1c) and 324.213(25)(b).
• Copies of Notarized Affidavit Forms that are signed and stamped are required. These are forms EQP4056 and EQP4057 for Qualified Consultants and the Person Submitting the report, respectively
